Mobility Action Program

A Program for Pacific people suffering from knee joint arthritis and gout.

K'aute Pasifika Services now provides a service specifically for Pacific peoples suffering from Osteoarthritis of the knee and or gout. This service called Mobility Action Program (MAP) is a Ministry of Health initiative that will see about 350 eligible clients accessing health service designed to bring comprehensive health care to clients and their families in their home and their communities.

The main bulk of the workload will be in Hamilton where about 250 clients will be enrolled and managed using the packages of care described below. A specialist nurse employed full time will lead the program in Hamilton and will be assisted by a Multidisciplinary team (MDT) to provide expertise in wellness programs (exercise and nutrition), physiotherapy, pain management, GP and specialist in rheumatology. In Tokoroa, responsible for about 100 clients, the set up will be a replica of the Hamilton one and will be managed by SWPICS.

 

Clients can access this service through their GP's, Waikato DHB referral coordination centre, other contracts including KPTs or SWPICS and self-referral.
