Chronic disease management

A chronic disease is an illness / disease that persists for a long time or is constantly recurring. These include long-term conditions such as hypertension (consistently high blood pressure), diabetes, lung disease, asthma and obesity related illnesses.

Chronic disease management services provide confidential support to improve and encourage people with long term conditions to self-manage their chronic illnesses and help them overcome some of the barriers that they struggle with.

Chronic disease management services focus on:

  • Education on how to manage your own health condition such as diabetes or blood pressure
  • Assessments
  • Individual care plans
  • Advocacy
  • Support to attend appointments and follow-ups
